MDS Coordinator (Level 1 & 2)
Facility: Lincoln County Care Center, Shoshone, ID Status: Non-exempt
Direct Supervisor: Administrator Indirect Supervisor:
Supervises: None
Scheduling Requirements: Normally Day Shift
Travel Requirements: Minimal to none

Job Summary
The primary purpose of the MDS coordinator is to maintain all medical records of residents and make sure that the documents adhere to both federal and state regulations. They also work closely with administrative and medical staff so as to keep resident records up-to-date and mostly handle the resident assessment instrument or RAI process for the patients from their admission up to discharge.

Job Responsibilities

The MDS Coordinator assumes daily responsibility in the following areas:

1. Develops treatment plans appropriate for patients that comply accordingly to MDS guidelines.
2. MDS coordinators are tasked in completing various MDS paper works; submit them on time and make certain it meets with the current guidelines and regulations as these regulations may vary each year.
3. Coordinate the completion of the resident assessment (MDS), Triggers and RAPs in accordance with current rules, regulations and guidelines
4. Ensure completion and submission of MDS data within regulatory timeframes
5. Coordinate the development of comprehensive assessments and care plans of each resident
6. Conduct/coordinate facility daily PPS meetings and IDT Process and Claim review
7. Assist with education and overall plan of care
8. Cooperate with monitoring and audit functions and investigations
9. Participate in the review Quality Indicator report and Resident Specific Profile
10. Participate in educational programs to maintain knowledge current with RAI/PPS regulations
11. Develop and maintain a good working relationship with inter-department personnel, as well as other departments within the facility, to assure that medical records can be properly maintained
12. Measure wounds when necessary
13. Prepare necessary reports for monthly CQI meeting.
14. Assist with investigations of incidents and accidents when necessary
15. Enter weights weekly
16. Attend and be prepared for all necessary meetings
17. Attend family conferences
18. Assist administrator with preparation of weekly meetings
19. Collaborate with other clinical staff and run assessment care plan meetings.
20. Other duties as assigned.

Essential Qualifications (Level 1)

1. Current licensure as a Licensed Practical Nurse in the state of Idaho
2. Ability to think and work independently or with direction, and communicate effective with other team members.
3. Firm knowledge of medical terminology.
4. Experience and knowledgeable in MDS 3.0

Preferred Qualifications (Level 2)
1. Current RAC certification.
2. Current NCPI certification.
3. Any documented experience as a MDS Coordinator (residential long-term care)
4. Any degrees, certifications or other qualifications to verify expertise
