Job Description
Family Health Services is seeking qualified candidates to fill a Diabetes Care Coordinator position. Under general supervision of the Director of Nursing, this position will provide outreach, navigation and coordination of care within the healthcare system for our patients with Diabetes and Hypertension with a focus on those who are uncontrolled. The Diabetes Care Coordinator is expected to exercise a high degree of initiative and judgment in coordinating patient care and follow-up.
Qualified candidates must demonstrate strong communication, organizational and time management skills. Bi-lingual English/Spanish skills required. Knowledge of local community resources preferred. Medical terminology experience preferred. Must be able to pass a background check and have a valid Idaho driver’s license.
